Purpose: This study used the Affective Events Theory to investigate the effects of caregiver attitudes toward caring for the elderly, emotional intelligence, job stress, and reciprocity on the quality of health care among caregivers in integrated nursing and care services. Methods: A cross-sectional survey design was employed. Participants included 147 caregivers working in integrated nursing and care service units at two hospitals in D City and one hospital in C City, with bed capacities ranging from 100 to 300. Caregivers were selected through convenience sampling in June 2024. The measurement tools used were the Attitude Toward Caring for the Elderly scale, the Wong and Law Emotional Intelligence Scale, the Job Stress Scale, the Nurse and Nursing Assistant Caregiver Reciprocity Scale, and the SERVQUAL (Service Quality) Scale. Data were analyzed using IBM SPSS/WIN 26.0. Results: Healthcare quality showed a significant positive correlation with career (r=.20, p=.018), attitudes toward caring for the elderly (r=.51, p<.001), emotional intelligence (r=.47, p<.001), and reciprocity (r=.68, p<.001), but a significant negative correlation with job stress (r=-.27, p=.001). Key variables influencing the quality of healthcare were reciprocity (β=.52, p<.001) and attitudes toward caring for the elderly (β=.19, p=.015), with an explanatory power of 51.0%. Conclusion: To enhance the quality of healthcare in integrated nursing and care services, it is essential to develop ongoing emotional education and training programs that improve job competencies and foster positive caregiver attitudes toward elderly care.
