Effectiveness of a Public Health Home Care Simulation for Nursing Students: Caring for Frail Older Adults

Purpose: This study aimed to examine the effects of a public health home care simulation for frail older adults the clinical practice skills, communication skills, critical thinking, and learner satisfaction of nursing students. Methods: A quasi-experimental design was employed with 82 fourth-year nursing students from two universities. All students received training on topics related to the public health home care simulation for frail older adults. The experimental group participated in a role-play simulation, while the control group engaged in a Nursing Lab virtual simulation. the WIN SPSS 25.0 program provided data analysis using chi-square tests, independent t-test, and paired t-test. Results: Both groups demonstrated significant improvement in clinical performance, and no significant difference was found between groups (t=0.66, p=.515). Although communication skills significantly improved in the experimental group (t=3.03, p=.004), there was no significant change in the control group (t=-0.75, p=.459). A significant difference in communication skill changes was observed between groups (t=2.73, p=.008). Critical thinking skills were significantly improved in both groups, and no between-group difference was found (t=-1.51, p=.136). There was no significant difference in level of program satisfaction between the two groups (t=0.69, p=.492). Conclusion: The results of this study suggest the necessity of developing a public health center home-visit simulation education program for all nursing students caring for frail older persons. Future research should focus on the development of a hybrid simulation education program that integrates virtual simulation and high-fidelity mannequins for frail older persons and the further evaluation of its effectiveness.
